Committed to keeping local youth in the game, Salem hospital is proud to offer free sports physicals to support athletes' health and safety. Sports physicals will be available for middle and high school students (6th - 12th grade)
When: Saturday, July 18, 2026
Time: 8:00 a.m. - 12:00 p.m.
Location: Salem Health Orthopedics 1600 State St, Salem OR 9301
What to bring:
- Parent or guardian (not required if youth is 15+ years old)
- Completed OSAA form (visit OSAA.org to complete) Copies will also be available at the event.
